Community Midwives
The midwives work independently as part of Derby Community Health Foundation Trust to provide care for new and expectant mothers. They attend the surgery for antenatal clinics at least twice a week.
Care Co-ordinator
Our Care Co-ordinator is part of the Community Support Team employed by Derbyshire Community Health Services.
Care co-ordinators provide a free service to anyone aged 18 or over to help support and act on your behalf as the link between all the different services and organisations in the community.
If you are an older adult the Care Co-ordinator can help you get the support you need to remain independent in your own home for as long as possible.
If you are a family member or carer of someone and you also feel you need some extra help, support or information – they can help with that too. They can also help you access local support and volunteer groups.
Health Visitors
The health visitors are based at Coleman Street Health Centre and can provide advice to expectant mothers and parents of young children. They can be contacted on 01332 861176. Derbyshire/County — Sara Lennon. Derby City — Amy Hawley.
Community Nurses
The local team of district nurses are not based at the surgery but at Coleman Street. They visit our housebound patients and those recently discharged from hospital at home.
